We use your information solely to determine whether you may qualify for a consultation with a spine specialist, to have a care coordinator contact you, and to improve our advertising and outreach. We do not sell your personal information.

Sharing

We share submitted information with the affiliated medical practice and care coordinators responsible for following up with you. We may share hashed, non-identifiable conversion data with advertising platforms (such as Meta) to measure ad performance.

Communications Consent

By submitting the form, you agree that a care coordinator may contact you by phone, text, or email about treatment options and scheduling. You can opt out at any time by replying STOP to texts or asking to be removed during a call.

Data Security & Retention

We store submissions in access-controlled systems and retain them only as long as needed for follow-up and recordkeeping.
